Your First Visit

Patient Forms

Upon entering the office, our receptionist will greet you and welcome you to our office.  She will then obtain a copy of your current health insurance card along with any other additional insurance information (personal injury, worker's compensation etc.) you may be utilizing to pay for your care while in our office.  Our office also requires a copy of current photo identification. We will then request you to fill out the New Patient paperwork if you have not already obtained the forms yourself online or by mail/fax.  Please arrive at the office 15-20 minutes prior to your appointment time to fill out this paperwork if you have not previously done so as our Providers are busy and run on a tight schedule.  The paperwork provides us with general information about yourself and your condition.  

As a reminder for our existing patients, we will also have to fill out update information forms if it has been a while since your last visit to our office or if your condition has recently changed.  Our providers want to be thorough and  provide you with the best possible care.  Our providers are better equipped to help you and  your particular health condition if we have update and accurate information including any hospitalizations, surgeries, injury/trauma (falls, sports injuries, accidents etc.) and changes in your condition.  The more information we have, the better we can serve you.  We thank you in advance for your patince and understanding.     


Consultation

Next you will have a consultation with one of our Doctors of Chiropractic to discuss your health-related problems, concerns, and potential treatment options. Your first visit is designed for your Doctor to learn more about you, your condition and your expectations to determine whether Chiropractic Care can meet your goals.


Examination

After your consultation, one of our Doctors of Chiropractic will perform a complete chiropractic examination testing your reflexes, your ability to turn and bend as well as other standard neurological, orthopedic, postural, and physical examinations. Nothing will be done in our office without your consent.


X-Ray Studies

Your specific condition may require us to take x-rays to get a full evaluation of your particular problem.  X-rays help us rule out more serious conditions and/or assist us in developing the most effective treatment plan for you.  All of our X-rays are read by a board certified Radiologist (M.D. and/or D.C.)  As a patient of Owensboro Wellness and Rehabiltation, you can have confidence that any abnormality or unique finding will be relayed to you by your particular healthcare provider.  And, we will make the appropriate referral to the appropriate specialist in a prompt and professional manner.


Report of Findings

Once all the information and examination have been performed, your Doctor of Chiropractic will give you a Report of Findings to explain the results and, most importantly, answer the three most popular questions: 

  1. Can you help me?
  2. How long will it take?
  3. How much is it going to cost?

After reviewing your particular history, examination, x-rays, and goals of treatment in our office, your Doctor of Chiropractic will discuss his/her best recommendations. If you have a condition that requires care with other providers, the doctor will tell you. If your Doctor believes that we can help you, we will begin your treatment program.


Treatment

Patients will generally be provided with their first treatment at the conclusion of the appointment. This may include spinal and/or extremity adjustments/manipulations, physical therapy modalities (electric muscle stimulation, moist heat, ice, therapeutic ultrasound etc.), and/or therapeutic exercises designed for your particular condition.  Treatment is interactive and we encourage communication.  Please express any concerns you have about the style of adjustment/manipulation, exercises, and therapies you receive while in our office.  Our goal is to provide you with a comfortable yet therapeutically beneficial experience while in our office.  And, if we need some special attention in a particular area, we want to know! 


Wellness Program

Upon release from active care, patients are instructed on a home exercise/stretching program for continued success and prevention of relapse of the condition.  Additionally, avoidance of certain activities or positions may be recommended.  If you desire an overall wellness lifestyle, our wellness team will work with you to create good habits and routines that support your success.  Every body is unique and different and therefore everyone requires a unique and customized wellness plan.  The foundation of our wellness lifestyle centers around prevention.  This includes: maintaining a healthy weight, eating a healthy diet, exercising regularly, maintaining proper posture and spinal alignment, vitamin and mineral supplementation when necessary, and stress reduction.
